Medicare and Insurance Funding

Medicare Funding 

To obtain one of our Communicator speech devices through Medicare, please complete the following steps...

  1. 1.Complete the “Client Information” form (click link below)

               View/Print “Client Information” form

  2. 2.Obtain a report/recommendation for a SGD (eg. Communicator U1) from 
    a Speech Language Pathologist (SLP)

               View/Print Sample SLP Evaluation Report (MedicareSLPSample.doc)

               Download Blank SLP Report (*.doc format)

  3. 3.Obtain a prescription for the SGD from the treating physician.

               View/Print Physician Prescription form

  4. 4.Photocopy both sides of your insurance card.

  5. 5.Fax all of all the above documents to 360-715-9633

Almost every day, we hear from people who have NO insurance, or have insurance that won’t pay for a speech device, or who don’t qualify for Medicare or Medicaid because of age or economic position.  And over the past 17 years, we’ve seen these calls increasing in number and frequency.  Added to this...Blue Cross has lowered the maximum it will pay toward a speech device to $1,000 and, the MDA (Muscular Dystrophy Assoc) has cancelled it’s $2,000 funding for speech devices beginning Jan. 1, 2010.

                                                           So today we decided to do something about it.


What if we offered our speech packages at prices that matched the out-of-pocket costs (20%) fully insured individuals spend on our competitors $8,800 speech packages?   And what if we made those prices available to anyone whether they had insurance or not?


                      In other words, remove the medical device “reimbursement max” ($7,100) out of the price completely.
